As a Social Prescribing Link Worker, you will support patients to improve their health and wellbeing by connecting them with non-clinical services and community resources. Working closely with GPs, the wider practice team, and local community services, you will provide tailored one-to-one support, empower people to take greater control of their lives, and contribute to reducing health inequalities.

Rockingham Forest PCN is a large, forward-thinking Primary Care Network compromising 3 practices across Corby, Kettering and Oundle.
This role is based across the Lakeside surgeries in Corby, Kettering and Oundle.

Lakeside Healthcare are looking to employ a Social Prescribing Link Worker to assist in the provision of care for patients within the Rockingham Forest Primary Care Network PCN geography.
The Social Prescribing Link Worker is a key member of the PCN multidisciplinary team, supporting the delivery of personalised care. The role places a strong emphasis on a what matters to me approach, ensuring that care and support are shaped around the individuals own priorities, values, and circumstances.
Social prescribing can help to strengthen community and personal resilience and reduces health inequalities by addressing the wider determinants of health, such as debt, poor housing, and physical inactivity, by increasing peoples active involvement with their local communities. It particularly works for people with long-term conditions (including support for mental health), for people who are lonely or isolated, or have complex social needs which affect their wellbeing.
Working collaboratively with GPs, healthcare professionals, and community partners, the Social Prescribing Link Worker will empower patients to develop the knowledge, confidence, and skills to manage their health and wellbeing in ways that are meaningful to them. In addition, the role will strengthen partnerships between primary care and local services, embedding person-centred, preventative approaches that reduce demand on general practice and contribute to the wider personalised care agenda within the PCN.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
