Job description

North Watford PCN are seeking a Social Prescribing Link Worker to join their team on a part-time basis. This role empowers people to take control of their health and well-being. A referral to a non-medical link worker is designed to support patients in being able to take a holistic approach, connecting people to community groups and statutory services for practical and emotional support.

Main duties of the job
  • Provide personalised support to individuals, their families, and carers to access community-based activities and support
  • Develop trusting relationships by actively listening to individuals concerns and motivations
  • Co-produce a simple personalised care and support plan and introduce people to appropriate activities, groups, and services
  • Help people identify the wider issues that impact on their health and well-being such as debt, poor housing, being unemployed, loneliness and caring responsibilities
  • Forge strong links with local VCSE organisations, community, and neighbourhood level groups, utilising their networks and building on whats already available
  • Work on your own caseload, updating relevant parties as required and referring the patient back to the GP or other suitable health professional if required.

North Watford PCN is a small innovative PCN delivering care to our 25,000+ registered patients and is a made up of three GP practices, Vine House, Abbotswood and Sheepcot Medical Centre. We are renowned for our excellent teaching reputation and have progressed well with the national recruitment agenda, already recruiting a GP, clinical pharmacist, social prescriber, paramedic, ANPs and a mental health link worker.
