A charity organization in the UK is seeking a Social Care Practitioner to join their team. This role involves assessing and supporting individuals with learning disabilities and autism, managing financial aspects of care, and collaborating with various stakeholders. This is a full-time position with a salary range of £24,000 - £28,840 per annum.

Working as part of a team, you will be one of the first points of contact between the public and the service, gathering information, identifying needs and completing assessments, and sign posting people to other services. You will work alongside qualified practitioners on a variety of tasks, whilst managing your own case load, assessing people’s needs and carrying out regular reviews under the relevant legislation.
The Social Care Practioner role will have a financial element that involves costing care packages for the social work team as well as chasing historically low care packages from the local authority. The successful candidate will have a competency with mathematics and the ability to develop knowledge around collating costed care and support plans.

One of the key purposes of this role is to ensure and maintain the high-quality service provision that the organisation offers. This role will support the facilitation of transition pathways for new admissions to the service, plus contribute and participate in meeting the needs of existing people in service.
