Job summary

South Gloucestershire Council is seeking an Advanced Social Care Practitioner to assess and recommend equipment and adaptations, promoting independence and participation in daily living activities. The role collaborates with Occupational Therapists, assistants, and social care practitioners to review and plan care outcomes.

You will manage risk, maintain high-quality reviews, and support day-to-day team operations.

Qualifications

  • Experience in statutory or voluntary sector work.
  • Understanding of safeguarding policies and procedures.
  • Ability to gather information, contribute to assessments and complete high-quality reviews.
  • Commitment to person-centred, values-based practice with strengths-based approaches.
  • Excellent communication skills and ability to engage with adults with varying needs.
  • Minimum 2 years experience in relevant area and NVQ Level 4 (or equivalent).

Job description

How you'll make a difference

As a Social Care Practitioner (OTA) or Advanced Social Care Practitioner in the Occupational Therapy Localities Team, you will assess and recommend appropriate equipment and adaptations to promote independence and support participation in activities of daily living.

You will support adults to maintain independence, safety and wellbeing within a collaborative, multi-disciplinary environment.

You will work along side Occupational Therapists and Occupational Therapist Assistants as well as Social Workers and Social Care Practitioners, playing a key role in reviewing care and support arrangements, ensuring plans remain outcome-focused, proportionate and compliant, while promoting choice and control for the people we support. The team’s core purpose is to deliver high-quality occupational therapy assessments that improve outcomes for people with care and support needs.

What you will be doing
  • It will be your responsibility to manage a caseload, ensuring timely, quality reviews, needs assessments and support planning.
  • Drawing on your experience and knowledge, you will review and adapt care and support plans to ensure they are strengths-based, proportionate, outcome-focused and promote independence, wellbeing and choice.
  • A key aspect of the role will be working collaboratively with a wide range of colleagues across different functions and partner organisations.
  • Using your professional judgement, you will manage risk and complexity appropriately, recognising and responding to safeguarding concerns in line with policies and procedures.
  • You will maintain accurate, up-to-date and confidential records, ensuring all information is recorded clearly and stored securely.
  • As an Advanced Social Care Practitioner, you will also support the day-to-day management of the team by ensuring staff have clear work programmes, receive appropriate support, and are provided with ongoing training and development opportunities.
What we need from you
  • It is essential you have experience in a relevant area of statutory or voluntary sector work andholdan NVQ Level 3 (or equivalent) in health, social care or a related field.
  • You will have a sound understanding of relevant legislation, policies and procedures, including safeguarding.
  • You will have the ability to gather information, contribute to assessments and complete high-quality reviews.
  • We are looking for someone who can commit to person-centred, values-based practice, aligned with strengths-based and proportionate approaches.
  • Given the nature of the role, you must have excellent communication skills, with the ability to build effective working relationships and engage confidently with adults with varying needs.
  • Additionally, as an Advanced Social Care Practitioner, youmust have a minimum of 2 years experience in a relevant area of statutory or voluntary sector work andhold an NVQ Level 4 (or equivalent) in health, social care or a related field.
What you need to know
  • To be considered for the Advanced Social Care Practitioner role, you must provide clear and detailed evidence within your application. The salary is offered within the following bands, dependent on experience:
    • Social Care Practitioner: £29,540 - £32,061
    • Advanced Social Care Practitioner: £32,597 - £35,412
  • You will be expected to travel throughout the authority and surrounding area so you must have a full, valid driving licence with regular access to a vehicle or have an alternative means of travel.
  • Business insurance will need to be added to your insurance policy and you can claim back mileage expenses.
  • Please note that due to the nature of this role, it is a requirement of employment that an enhanced Disclosure and Barring Service (DBS) check is obtained.
  • This role is hybrid, with a mixture of working at our office in Badminton Road, working from home and working across South Gloucestershire localities.
