Reablement - Empowering Independence

Reablement is a short-term, therapist‑led service that supports people to stay independent, avoid unnecessary care, and live well at home. We work closely with individuals, their families, and community partners to build personalised support plans that focus on what matters most to them.
As an Experienced Social Care Practitioner, you’ll carry out assessments, develop support plans, and review outcomes in line with the Care Act 2014. You’ll work with people to understand their strengths, identify risks, and connect them with local resources. You’ll also play a key role in safeguarding, supporting carers, and promoting wellbeing.
You’ll need experience in a health or social care setting, strong communication skills, and a good understanding of relevant legislation. A Level 4 qualification in Health and Social Care (or equivalent) is ideal, but we’ll support the right candidate to achieve this.

Job responsibilities
- To undertake person‑centred work with customers and plan for the provision of support and/or services to help them meet their outcomes and live as independently as possible, considering whether they need support to fully participate in the assessment and support planning process.
- To consider whether there is a concern about an individual’s capacity to make a specific decision, including restrictions placed on an individual which may amount to deprivation of their liberty (continuous supervision and control).
- To write support plans for individuals that reflect their needs and how they will be met, in a manner that complies with Care Act requirements.
- To have a good understanding of assessment, support planning and risk management, and apply Council policy to support customers in managing risk appropriately.
- To carry out support plan reviews in a timely and person‑centred way, as required and identified by departmental targets, assessing whether agreed outcomes have been met.
- To gather sufficient information about customers to accurately assess their needs against the national minimum eligibility criteria within the Care Act 2014, including working creatively to identify sources of support to improve wellbeing and independence, and determining eligibility for support from Wiltshire Council.
- To identify where adults may be at risk and act in accordance with safeguarding policies and procedures.
- To support carers to continue in their role by identifying their needs for support, referring them to Carer Support Wiltshire for assessment, and considering the impact on the whole family.
- To record all information and activity accurately and timely in the social care records database, meeting work targets set by the Team Manager and Supervisor.
- To gather and update local knowledge about universally available services, including voluntary and community sector options, commissioned and contracted services, and to signpost/advocate for customers accordingly.
- To evidence need for continuing support, identify promotion of increased independence, and collaborate with customers and care providers.
- To represent the Council at inter‑professional and inter‑agency meetings, such as MDT meetings.
- To contribute proactively to effective team working with a positive attitude, sharing knowledge, offering advice and support, and being prepared to develop services.
- To engage and prepare for supervision and appraisal through self‑evaluation, ensuring continual professional development is evidenced.
- To be an ambassador for the Council and department, always representing the Council positively, professionally and appropriately at meetings.
- To attend Civil Emergencies when a Rest Centre is set up (rare).
