Job summary

The London Borough of Newham is seeking a Social Care Officer to carry out needs assessments and reviews, promoting independence and Direct Payments within the Adult Social Care neighbourhood team. You will work with customers and carers to deliver effective case work.

We are looking for candidates with strong assessment and care planning skills who are passionate about delivering results and building strong links with community agencies.

Qualifications

• Experience with Care Act assessments and promoting independence.
• Ability to carry out needs assessments, reviews and care planning.
• Strong collaboration with customers, carers and community agencies.

Responsibilities

• Undertake assessment of needs under the Care Act and develop care plans.

Social Care Officer

We are looking for 1 Social Care Officer who has and wants to further develop their skills with case work under the Care Act like promoting independence and Direct Payments, carrying out assessment of needs, reviews and working with customers and carers in the community and building good links with community agencies in the neighbourhood team within Adult Social Care. We are looking for Social Care Officer who have good assessment and care planning skills and passionate about delivering results.

Salary

£39,519- £40,575

About Us

The London Borough of Newham is located only 5 miles east of London. It is well positioned with access to all major networks in London and the south east.
