Portsmouth City Council is seeking a proactive professional to lead the Emergency Department service at Queen Alexandra Hospital. The role supports safe hospital discharges and reduces unnecessary admissions through person-centred, strength-based interventions.
You will work with health and social care colleagues, using digital tools to input data, assess risks, and apply safeguarding principles across Portsmouth. Band 7, £33,119 - £37,562 p.a, UK Right to Work required.
The post is based within the Portsmouth Adults Intermediate Care Team (AICT) supporting our integrated services including Community Rehab Service, The Transfer of Care Hub, The acute hospital and Inpatient units. We are seeking an enthusiastic professional to take the lead in our exciting Emergency Department service based at Queen Alexandra (QA) Hospital. This pivotal role will work closely with health and social care to support timely and safe hospital discharges, prevent unnecessary admissions, and ensure individuals receive the right support at the right time. You will provide information, advice, guidance within a busy acute hospital environment, supporting to deliver person-centred, strength-based interventions. As this is a SCAP post and depending on the needs of the service you may be required to work in other areas/locations in the business when needed.
The protection and safeguarding of adults with care and support needs is also an integral part of the role, requiring commitment to making safeguarding personal. You will work autonomously and take responsibility for organising and managing your work. You will need to be self-motivated and resilient, able to work collaboratively and communicate clearly with a variety of people in different settings. You will understand risk and put in place effective plans to support people and manage risks, including where people lack capacity to make decisions for themselves.
You will use a variety of digital technologies and be able to input data and analyse information to support you in your work
You will need to have experience of working with adults with care and support needs, their families and carers. You will have a level 3 to 4 qualification in a relevant field or be able to demonstrate high levels of transferable knowledge/skills. With experience of report writing and using digital systems to record information, you will be able to work autonomously and take responsibility for organising and managing your work. Self-motivated and resilient, you will be able to work collaboratively and communicate clearly with a variety of people in different settings.
You will need to be able to understand risk and put in place effective plans to support people and manage risks, including where people lack capacity to make decisions for themselves. You will have knowledge of the relevant statutory framework applicable to social care and an understanding of strengths based and person-centred practice and the ability to apply this in practice.
In addition, you will have the ability to travel around the city and gain access to a variety of venues throughout Portsmouth including clients homes.
