Position

We are currently seeking a full time day Skill Builder I for our Adult SMI program located in Richmond, IN. This position will provide skills and development training to assist patients and/or families in developing skills directed towards achieving developmental milestones and reducing psychosocial barriers. A Skill Builder provides case management services, working in conjunction with the clinical team for coordination of care. A Skill Builder works with the patient on a day to day basis using professional judgment and discretion to implement the team determined treatment plan.

Responsibilities
  • Assists the Therapist in developing, implementing and revising individual treatment plans; assures that services provided are specified in the treatment plan and monitors progress toward treatment goals.
  • For adults, provides individual and group skills training and development to assist the patient and/or family in developing skills (i.e. Self-care, daily life management, interpersonal communication, parenting, illness management, anger management, positive coping skills, and problem solving skills) directed toward reducing psychosocial barriers.
  • For children, provides individual and group skills training and development to assist the patient and/or family in developing skills (i.e. Coping with body image changes, coping with hormonal-induced emotional mood swings, self-care, daily life management, interpersonal communication, illness management, anger management, positive coping skills, and problem solving skills) directed toward achieving developmental milestones and reducing psychosocial barriers.
  • Provides case management services as identified in the patient’s treatment plan.
  • Works collaboratively with the Therapist and medical staff to ensure coordination of integrated care for the patient, including identification of health issues, compliance with treatment, and effective patient to medical provider communication.
  • Completes CANS/ANSA re-assessments.
  • Provides consultation to staff on specific cases involving coordination of care.
  • Maintains positive working relationships with staff of other agencies and institutions, homes, and facilities, and acts as a liaison between patients and agencies and/or institutions.
  • Prepares and maintains clinical records, reports and forms and other administrative tasks as required.
  • Participates in continuing education activities, remaining knowledgeable in areas of expertise.
Qualifications
  • High school diploma or its equivalent with 2 years paid behavioral healthcare experience or equivalent college coursework toward a human services or health related degree required at minimum
  • Associate or Bachelor's degree preferred
  • A valid driver's license with a driving record meeting Meridian's driving policy, reliable transportation, and proof of auto insurance
  • A commitment to providing individual attention to help clients achieve mental, emotional and physical well-being
