Job summary

Pennine Care NHS Foundation Trust in Rochdale is seeking a Band 6 Community Mental Health Practitioner to work within Older Adult Services. You will screen referrals from primary and secondary care, assess in the person’s home, and decide on the most appropriate service to meet needs.

The role involves autonomous decision making, holistic assessments, risk management and coordinating with a multidisciplinary team to deliver community-based care.

Qualifications

• Registered RMN with current UK registration.
• Post-qualification experience in mental health.
• Strong communication skills and MDT working.

Job summary

This is an exciting opportunity for a Band 6 Community Mental Health Practitioner working within Older Adult Services in Rochdale. The role involves screening referrals which are received from primary and secondary care services and making decisions on the most appropriate service to meet the needs of the person. • To involve family and carers in the assessment process with service users consent
