Job description

Support Worker - Short Term Services

We are recruiting for 3 Support Workers to join our team on a part-time basis (36 hours and 2 x 30 hours) on a permanent contract. You will provide care and support to individuals in the community, focusing on maximising independence and delivering high-quality care, including end-of-life care.

Following acute illness or hospitalisation, you will undertake short-term care as outlined in care & short-term service plans or delegated tasks from Therapists/Nurses. You will contribute to assessments, monitor progress, review goals, and report any changes in medical conditions.

This role supports our initiative to prevent avoidable hospital admissions by working alongside intermediate care teams and other community services. You will work as part of a team and may also work alone within the community, across Torbay and South Devon, collaborating with other organisations to provide integrated health and social care.

Responsibilities
  • Support service users with personal, domestic, social, and clinical needs according to care plans, within your competencies.
  • Observe and evaluate responses to care plans, communicating any changes to the relevant team members promptly.
  • Offer flexible support across shifts between 8:00 am and 10:00 pm, 7 days a week.
  • Complete reports on service users using the IT system, adhering to Trust policies.
  • Maintain a safe working environment and report issues in line with policies.
  • Undertake relevant training and personal development activities.
  • Provide support to other teams during periods of reduced workload.
Why Work With Us

The service operates 7 days a week, from 8:00 am to 2:00 pm and 4:00 pm to 10:00 pm. We provide full training, a uniform, and paid shifts regardless of contact time. Travel time is fully paid, with a generous mileage allowance. Shift enhancements are available for evenings, weekends, and bank holidays.

There are opportunities to rotate through different settings and progress your career, such as into an assistant practitioner role. Located in Torbay and South Devon, the area offers access to the Moors and coastline, making it a great place to work.

Our team values flexibility and work-life balance, offering benefits such as:

  • NHS Pension Scheme
  • Childcare at our nursery
  • Generous holiday entitlement starting at 27 days plus bank holidays
  • Staff discounts
  • Career development and training opportunities
  • 24-hour employee support helpline
