We are recruiting for 3 Support Workers to join our team on a part-time basis (36 hours and 2 x 30 hours) on a permanent contract. You will provide care and support to individuals in the community, focusing on maximising independence and delivering high-quality care, including end-of-life care.
Following acute illness or hospitalisation, you will undertake short-term care as outlined in care & short-term service plans or delegated tasks from Therapists/Nurses. You will contribute to assessments, monitor progress, review goals, and report any changes in medical conditions.
This role supports our initiative to prevent avoidable hospital admissions by working alongside intermediate care teams and other community services. You will work as part of a team and may also work alone within the community, across Torbay and South Devon, collaborating with other organisations to provide integrated health and social care.
The service operates 7 days a week, from 8:00 am to 2:00 pm and 4:00 pm to 10:00 pm. We provide full training, a uniform, and paid shifts regardless of contact time. Travel time is fully paid, with a generous mileage allowance. Shift enhancements are available for evenings, weekends, and bank holidays.
