Kent County Council is seeking a Social Care Case Officer for the East Kent Short Term Pathways Team. You will assess adults over 18 with diverse needs recently discharged from hospital, and develop care and support plans to promote independence.

The role involves travel across East Kent, a full UK driving licence, and collaboration with health colleagues and partner agencies. A comprehensive induction and ongoing professional development are provided.
