Job summary

Walsall Council is seeking a motivated Shared Lives Officer to support adults with a range of needs in family-based placements. You will assess, monitor and review cases, work with carers and health professionals, and promote independence and wellbeing within a local community setting.

The role requires strong communication skills, partnership working, and the ability to develop person-centred plans. A DBS process and flexible hours may apply.

Qualifications

• NVQ Level 3 in Adult Social Care or equivalent qualification.
• Experience with vulnerable adults in a care setting.
• Knowledge of safeguarding and person-centred approaches.
• Ability to work in partnership with carers, families and agencies.

Responsibilities

• Support assessment, review and ongoing monitoring of individuals.
• Work with Shared Lives Carers to ensure safe, appropriate placements.
• Develop and review person-centred support plans.
• Maintain accurate records and case management information.
• Contribute to recruitment and promotion of Shared Lives carers.
