We have an exciting opportunity for a Service Team Manager to lead our Hospital Discharge Team!

Main duties of the job

As Service Team Manager for the Hospital Discharge Team, you will lead a multidisciplinary team working across acute settings to ensure safe and person-centred discharges. You will:

  • Provide visible, supportive leadership to staff, promoting resilience and reflective practice.
  • Coordinate effectively across health and social care partners to advocate for individuals' rights and needs.
  • Ensure compliance with statutory duties including the Care Act, Mental Capacity Act, Continuing Health Care, and safeguarding.
  • Champion strengths‑based and person-centred practice in a fast‑paced, multidisciplinary environment.
  • Contribute to RBWM's strategic priorities: building inclusive, healthy communities, promoting prevention and choice, and delivering compassionate, high‑quality services.

Job responsibilities

We are recruiting a knowledgeable and compassionate Service Team Manager to lead our Hospital Discharge Team at the Royal Borough of Windsor & Maidenhead. This important role is central to delivering our Social Care Futures vision and ensuring that hospital discharges are safe, timely, and person-centred.

This is a full-time, permanent role with hybrid working, and a salary of £53,296 – £58,286 per annum.

Person Specification
Qualifications
  • Be a professionally qualified Social Worker holding either CQSW, DiPSW, Degree in Social Work or equivalent.
  • Registered with Social Work England.
  • PEPS2 qualified and experienced practice educator (desirable).
  • Qualified as a Best Interest Assessor (desirable).
Experience
  • Substantial experience in adult social care (minimum 3 to 5 years), ideally including roles as a Senior Practitioner or Assistant Team Manager or Team Manager.
