A humanitarian organization is seeking a Service Coordinator to lead a team in assisting patients transitioning from hospital to home. This role involves managing referrals, coordinating volunteers, and ensuring high-quality care. Service Coordinator - Assisted Discharge
Location: Based at the Royal Bournemouth Hospital with occasional work at Poole Hospital
Salary: £23,590 to £25,155 per annum, pro rata
Hours: 21 per week, (Mon, Thurs & Fri 8am-4pm)
Contract: Permanent
Additional Requirement: Applicants must have a full manual UK driving licence (2 years)
