Service Coordinator - Assisted Discharge

Location: Based at the Royal Bournemouth Hospital with occasional work at Poole Hospital

Salary: £23,590 to £25,155 per annum, pro rata

Hours: 21 per week, (Mon, Thurs & Fri 8am-4pm)

Contract: Permanent

Additional Requirement: Applicants must have a full manual UK driving licence (2 years)

As a Service Coordinator, you’ll be the vital link between hospital referrals and care at home. You’ll manage a team of staff and volunteers, ensuring services are delivered safely, efficiently, and with heart. This role is central to our mission of connecting human kindness with human crisis.

What will a day in the life of a Service Coordinator involve?
  • Acting as the main point of contact for hospital and community referrals.
  • Coordinating and supporting a team of staff and volunteers to deliver services.
  • Monitoring service delivery, maintaining accurate records, and ensuring quality standards are met.
  • Attending hospital meetings, managing the service budget, and stepping in to support frontline delivery when needed.
