Service Coordinator - Assisted Discharge

Location: St Mary's Paddington, covering London

Salary: £23,590 to £25,155 per annum, pro rata plus ILW of £3,344, if residing & working in London

Hours: 35 per week (Mon-Fri)

Contract: Fixed term contract until 30th September 2026

As a Service Coordinator, you’ll be the vital link between hospital referrals and care at home. You’ll manage a team of staff and volunteers, ensuring services are delivered safely, efficiently, and with heart. This role is central to our mission of connecting human kindness with human crisis.

What will a day in the life of a Service Coordinator involve?
