Job summary

A prominent healthcare trust in the United Kingdom seeks a Senior Urgent Care Triage Practitioner. You will play a critical role in triaging patients and coordinating care within a multidisciplinary team. Qualifications

  • Evidence of working at band 5 level within the community or acute setting.
  • Knowledge of remote monitoring and available technology.
  • Interest in learning new skills for remote clinical monitoring.

Responsibilities

  • Triage patients referred to the UCCH as part of the multidisciplinary team.
  • Assess and prescribe care pathways for patients in crisis.
  • Work within the integrated team to prevent unnecessary admissions.

Education

Current NMC registration or Current HCPC registration
