The Confederation, Hillingdon CIC is seeking a GP to provide senior clinical oversight within the Reactive Care Coordination Hub for a one-year fixed-term post. You will perform rapid assessments of urgent referrals, decide on pathways to avoid admission and support a two-hour morning handover, ensuring coordinated care across UCR, Virtual Ward, D2A and YLL services.

Applicants must hold GMC registration and be a qualified GP, with extensive experience in urgent care and virtual consultations.

Responsabilidades

• Provide senior clinical oversight for patients referred to the Reactive Care Coordination Hub.
• Carry out rapid assessment and clinical decision-making to avoid hospital admission.
• Support the two-hour morning clinical handover and coordinate care pathways.
• Review and interpret diagnostic results to guide safe, evidence-based decisions.
• Coordinate with GPs, Hub Consultant and community providers to optimise care.
• Record decisions and outcomes in line with SOPs and systems; contribute to audits.
