As a member of our team, you'll play a pivotal role in supporting individuals in transition within our Move on Housing portfolio. You'll provide crucial medium to low support, helping clients move from high support temporary services to independent living.

Skills We're Looking For:
  • Experience: Previous experience in delivering support to individuals with complex and enduring needs, and the use of risk assessments/support planning tools
  • Knowledge: Good knowledge of mental health wellbeing, addiction, and relapse management are valued
  • Communication: Clear and compassionate communication is key to success in this role.
  • Commitment: Dedication to promoting equality, diversity, and inclusion in all aspects of your work.
  • Post holder must hold a clean driving licence and be able to drive a vehicle.
