Job description

This role will provide you with the opportunity to deliver a high standard of care to patients in their homes alongside developing your leadership skills supporting your team.

The successful candidate will work autonomously and as part of a team contributing to the provision of 24/7 Specialist Palliative/End of Life Care. They will be responsible for not only providing care, but for providing advice and support to patients, their families and other healthcare professions within the community.

You will provide support to patients in their own homes or preferred place of care who have palliative or end of life care needs, raising clinical standards and promote a learning environment for other health care providers. They will be responsible for planning, assessing, implementing, and evaluating care in partnership with the Multidisciplinary Teams and out of hours providers.
