Job Title: Senior Social Worker – Hospital Discharge Team

Location: Kingston

Rate: £32.35

Liquid Personnel is currently recruiting for a Senior Social Worker to join a busy Hospital Discharge Team based in Kingston.

Your responsibilities

You will take personal responsibility for continuing professional development and maintaining performance levels. You will manage your time effectively and respond flexibly to service needs, provide support and cover for the Team Manager and / or Principal Manager, and actively participate in the performance management system, including supervision, PAR, team meetings, and service reviews. You will follow departmental health and safety procedures, develop skills through work experience in various divisional situations such as secondments, job rotation, and shadowing, and contribute to information management processes. Additionally, you will support social work students on placement under the direction of the Principal or Practice Manager, acting as a workplace supervisor or Practice Teacher if appropriately trained. You will also need to understand and integrate into practice the key policies of the council in relation to social care with children and families, especially multi-disciplinary working, integration, and whole family approaches.

Qualifications and Experiences

You must have knowledge and experience of hospital discharge, good IT skills, and knowledge of LAS / IAS and Google. You should be comfortable completing Care Act assessments after the D2A period.
