Manage a caseload of complex cases for young people aged 16 to 25, conducting assessments and pathway planning in line with the Children (Leaving Care) Act.
Empower young people by championing their voices and co‑producing plans that reflect their aspirations.
Lead safeguarding assessments and collaborate across multi‑agency teams on high‑risk cases.
Support young people in developing resilience and independence, ensuring successful transitions into education, employment, and secure accommodation.
Provide practice leadership, mentoring Social Workers and Personal Advisers, and overseeing quality assurance processes.
