As a Senior Social Worker, you will provide expert guidance, risk management, and decision-making within adult care home pathways. You will work collaboratively to support safe, sustainable placements and uphold the rights and wellbeing of adults with complex health and social care needs.
Undertake strengths-based assessments in accordance with the Care Act 2014, promoting independence, wellbeing, and person-centred outcomes.
Manage a complex and varied caseload, including individuals with dementia, learning disabilities, mental health needs, and other vulnerabilities.
Lead safeguarding enquiries under Section 42, ensuring robust decision-making and timely protection planning.
Provide supervision, guidance, and mentoring to junior staff, supporting team development and best practice.
Work closely with care home providers, NHS partners, commissioners, and other stakeholders to ensure high‑quality, sustainable care arrangements.
Fulfil statutory responsibilities, ensuring compliance with legislation including the Mental Capacity Act, Care Act, and associated guidance.
Maintain accurate, reflective, and timely case records, ensuring all interventions meet statutory timescales and quality standards.
