Norfolk and Suffolk NHS Foundation Trust (NSFT) is seeking a qualified social worker to join our multidisciplinary team. You will plan and deliver direct patient care in community or hospital settings, participate in assessments, triage and care planning, and provide crisis intervention.
The role involves safeguarding responsibilities, adherence to mental health and safeguarding legislation, and contributing to student learning environments within NSFT.
The post holder will undertake the planning and delivery of direct patient care within a community mental health or hospital setting and to discharge statutory community care functions on behalf of the organisation. To be an active member of the integrated multi-disciplinary team, ensuring the provision of social care services, participating in assessment, triaging, care planning, treatment, evaluation, and crisis intervention/management. To deliver the provision of Mental Health Care services to people with mental health care needs and provide support, advice and guidance to other health and social care professionals. Provide an appropriate learning environment for relevant students.
