Key Responsibilities

• Manage post hospital pathways including HomeFirst and step-down beds.
• Manage and attend system calls and provide regular system updates.
• Supervise Social Workers and unqualified staff, providing advice and support to the team.
• Undertake Care Act assessments and Mental Capacity Assessments (MCA).
• Apply safeguarding knowledge to ensure safe and appropriate decision making.
• Work collaboratively with health and social care partners to achieve safe and timely discharges.
• Maintain accurate and up-to-date case records in line with legislation and local authority policy.

Essential Requirements

• Degree/Masters in Social Work and SWE registration.
• 2+ years of post-qualification experience.
• Strong knowledge of UK social care legislation and practice including Care Act, MCA, and Safeguarding.
• Experience in hospital discharge pathways.
• Right to work in UK.
