What you will do
  • Conduct swift and thorough initial assessments to facilitate the safe discharge of service users from the Royal London and neighbouring hospitals.
  • Utilise the Discharge to Assess Model to effectively manage high turnover of assessments and caseloads.
  • Collaborate with a supportive team in a hybrid work environment, transitioning cases to appropriate locality teams post-discharge.
  • Maintain close communication with hospital staff, families, and external agencies to ensure a seamless discharge process.
  • Work flexibly within the schedule of Thursday 3pm to 8pm, Friday to Sunday from 9am to 8pm.
