Overview

This post is to support the Hospital at Home service which is a collaboration with the acute trust to deliver hospital standard treatment within a patients’ home. The Hospital at Home philosophy is providing acute clinical care at home which would otherwise be provided in a hospital.

The service is part of the Urgent Care Services which provides a two-hour response to patients in crisis at home. Our aim is to prevent unnecessary hospital admission and facilitate early discharge from the acute hospitals. We also provide unscheduled care out-of-hours for patients who may be at the end of life, or experiencing an urgent need for healthcare. This may include treating acutely unwell people that would otherwise require hospital admission, and also supports hospital discharges. The service convers 24 hours a day 365 days a year.

Nurses use a wide clinical skills base to provide hospital-level care at home. This includes IV therapy, end of life symptom control and management via syringe driver, catheterisation skills, phlebotomy, wound management, risk assessment, as well as general observations and fundamental nursing care.

This service is providing nursing care within patients’ homes and covering West Kent. The ability to drive is essential to this post.

Responsibilities
  • Assessment, planning, implementation and evaluation of holistic care
  • The development of integrated care plans in partnership with the patient
  • To order equipment and send onward referrals as necessary
  • To act as a lead and senior practitioner to support junior staff to carry out assessments/reassessments of service users and/or pathways with complex needs across the relevant field.
  • To actively work as a member of the multi-professional team to provide high quality care to patients
  • To undertake a range of competent clinical interventions and identify any deterioration in patients’ condition, taking appropriate action.
  • Manage a caseload of patients on a daily basis consisting of patients with complex needs.
  • To support in duty role and complete triages
  • To support the Clinical Lead in the management of the team
  • To support and make recommendations to senior managers in order to prioritise and focus efforts to drive forward and achieve business objectives, service improvement strategies and statutory requirements.
