An exciting opportunity has become available for a 10 month fixed term/secondment opportunity with the Durham Tees Valley Adult Learning Disability Physical Health Team as a Senior Registered Nurse / Health Facilitator.
The aim of the DTV ALD Physical Health Team is to break down barriers, to improve health outcomes and improve quality of life for people with learning disabilities.
Candidates must have 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework at the time of applying.
Applicants must ensure that prior to applying they have agreement from their current line manager that they can be released on a secondment basis. In the event that permanent funding is secured for this post the successful applicant will be confirmed in post on a permanent basis.
The successful candidate will work with health colleagues within the team and wider learning disability community teams, local authority and in liaison / partnership with primary and secondary care services, using a person-centred approach to enable and support adults with a learning disability and their family and carers to promote the health needs of people with a learning disability and enable access to wider health services.
