If you think you would be able to work in a high-pressured setting, delivering all aspects of clinical care, then this could be the role for you.
We are looking for Registered Adult Nurses with senior experience at Band 5 to join us as part of the Primary Care team within the prison of HMP High Down. We are able to offer supervision, training and preceptorship. You do not need to have prison experience as we will provide you with a full induction and training.
Please note that any candidate wishing to apply for a role within our team will have to complete prison vetting clearance. This requires that you have been a resident in the UK for a minimum of 3 years.
Responsibilities
- Primary care is responsible for screening all new arrivals, health promotion, long term condition management, minor ailments and responding to incidents wherever they may occur in the prison.
- Work alongside the Mental Health and Substance Misuse team to provide holistic care addressing physical and mental well-being in partnership.
- Collaborate with GP, dentistry, optician, podiatry, physio and radiology services, and work closely with HMPPS staff, education and other professionals.
- Contribute to safeguarding vulnerable patients in line with CNWL Trust safeguarding policies.
- Apply local risk assessment and management protocols; make appropriate referrals to other services for long term or complex care management.
- Coordinate multi-disciplinary clinical practice to provide focused and integrated care programs/packages.
- Utilise highly skilled interventions to de‑escalate aggressive patient behaviours and follow appropriate processes for handling them.
- Work collaboratively with prison, education staff and other disciplines to provide integrated and coordinated care.
- Ensure training needs are communicated to the team manager and participate in local and trust audits, clinical governance and research within the area served by the multi-disciplinary team.
- Liaise with other professional services (hospital staff, GP, caseworkers) sharing information where appropriate.
- Maintain high standards of clinical record keeping, including electronic data entry and recording.
- Attend prison meetings as required, contribute to clinical issues, and ensure actions are communicated to the appropriate responsible individuals.
- Create pathways of care for patients, including collecting collateral information and making referrals into the community when discharging.
- Maintain professional communication about risk and ensure sensitive and comprehensive information sharing.
- Maintain professional practice standards and ensure diversity considerations are reflected in care delivery.
Research, Teaching and Supervision
- Utilise theory, evidence-based literature and research to support practice; assist in audits as part of the annual plan to improve service provision.
- Participate in regular clinical and managerial supervision; attend business and clinical meetings and reflective practice groups.
- Develop skills and competencies to support current duties; contribute to training and support of all staff; manage and supervise nursing staff; ensure appraisals and development plans are up to date.
- Develop nursing skills and systems for effective triage, minor ailment management and appropriate referrals to other healthcare services.
- Participate in induction and training of new staff, peers, junior staff and students.
Management, IT and Policy
- Assist in design and implementation of service development projects; attend meetings where service developments are planned and discussed.
- Input and maintain electronic clinical records and statistics; provide data to the Service Lead/Performance team when necessary.
- Work in accordance with CNWL/Health & Justice Service Directorate policies, with the NMC Code of Conduct and Guidance on Professional Practice; stay up to date with policies and documents.
- Work with Health & Justice Service Directorate procedures, including security and health and safety; apply equal opportunity principles and participate in training activities and performance reviews.
General
- Contribute to maintaining high professional standards through internal and external CPD activities, in consultation with the manager(s).
- Maintain high standards of clinical record keeping and report writing in line with professional codes of practice and Trust policies.
- Perform other duties of a similar kind to the grade as required by the manager.
