Qualifications

A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
Current registration with Social Work England.
Significant experience working with young adults, particularly in transition settings.
Strong knowledge of relevant legislation, policies, and best practices in social work.
Excellent communication and interpersonal skills, with an ability to engage and motivate young people.

Responsibilities

Developing and implementing personalized transition plans for young adults.
Providing expert advice and support to social workers and other team members.
Conducting assessments and evaluations to identify risks and opportunities for young adults in transition.
Building strong relationships with young adults, their families, and external agencies.
Monitoring and reviewing progress to ensure interventions are effective and aligned with best practices.
