At Peterborough City Council's Transfer of Care Team, we believe in the power of change and the importance of community. We've got some fantastic opportunities due to recent promotions, and we're on the lookout for enthusiastic senior social worker to join our friendly, fun, team!
Our team provides essential support for adult service users, ensuring safe and timely discharges from hospital. Working closely with Peterborough City Hospitals multi-disciplinary teams, you will enable discharges within the Discharge to Assess framework.
