A local government authority in Peterborough is seeking an enthusiastic Senior Social Worker to join their Transfer of Care Team. This role involves providing essential support for adult service users during hospital discharges, collaborating with multi-disciplinary teams. Our team provides essential support for adult service users, ensuring safe and timely discharges from hospital. Working closely with Peterborough City Hospitals multi-disciplinary teams, you will enable discharges within the Discharge to Assess framework.
The Transfer of Care Team is supportive, it offers a positive working environment where development is encouraged. You will be part of a collaborative, leadership team, promoting exceeptional quality and practice.
Excellent knowledge of current legislation alongside strong motivational skills. The ability to be able to work and support the team whilst also being able to make informed decisions utilising your own judgement.
