Job description

An exciting opportunity has arisen for an individual to join our expanding Greater Manchester Reconnect service, in the role of Senior Practitioner.

The post holder will have excellent communication and person skills and have extensive experience of working with individuals in mental health, learning disability or criminal justice services.

Our Greater Manchester Reconnect Service is commissioned by NHS England and is a care after custody service that seeks to improve the care of people leaving prison by offering our service and support pre-release, and peer led navigation and support in the community post release.

The Greater Manchester Reconnect Service will be provided in all Greater Manchester prisons and the Greater Manchester community.

Main duties of the job

The post holder will work within a multi-disciplinary team responsible for supporting individuals leaving the prison setting to enhance their health, wellbeing and quality of life, assisting them to connect with and access health services, voluntary sector services and community-based activities and support.

The post holder will work agilely across the GM footprint to:

  • Provide supervision, support and advice to junior staff in the team, in line with relevant clinical pathways as outlined within the operational procedures.
  • Manage and evaluate a caseload in line with relevant pathwaysas outlined within the operational procedure and will include: -

Person Specification
Education / Qualifications
  • RMN, RNLD, OT, SW with current professional registration.
  • Post registration training in relevant field
  • Leadership/Management training
Experience
  • Experienced mental health, CAMHS or learning disability practitioner with a good working knowledge of how health and social care services are delivered in a community setting.
  • Experience of Multi-Disciplinary working
  • Evidence of positive contribution to change management
  • Demonstrate experience of problem-solving approaches
  • Experience or knowledge/understanding of the Criminal Justice system and settings.
  • Experience of working with young adults
  • Experience of working with women
Knowledge
  • Awareness of children/vulnerable adults' guidelines
  • Knowledge or willingness to develop awareness of wider mental vulnerability.
  • Knowledge of risk assessment tools in mental health.
  • Understanding of the interface between health, social care and criminal justice services and a commitment to overcoming potential barriers.
  • Knowledge of the wider criminal justice agencies and their role in risk management.
  • Awareness of criminal justice legislation.
