We have a fantastic new opportunity for a Senior Practitioner / Community Health and Social Care Coordinator Integrated Neighbourhood Team Lead to support adults and their carers across Rutland, promoting resilience and social inclusion within the community.

SALARY: £44,075 to £47,181 per annum + Benefits (see below)

LOCATION: Hybrid Working / Oakham, Rutland, East Midlands

JOB TYPE: Full-Time, Permanent

WORKING HOURS: 37 Hours per Week. Flexible and Agile working

DUTIES
  • Lead and Supervise Team: Manage Integrated Care Coordinators and Community Inclusion Workers through regular supervision and development
  • Coordinate Complex Cases: Ensure professional involvement supports the needs of adults at risk and their carers
  • Develop Integrated Neighbourhood Model: Build partnerships across health, social care and voluntary services to deliver coordinated support
  • Promote Community Resilience: Support individuals to connect with their communities and live safely and independently
  • Provide Specialist Advice: Offer guidance to colleagues on complex cases and safeguarding matters
  • Monitor Performance: Support service reviews, data collection and quality assurance processes
  • Manage Budgets: Ensure resources are used efficiently and in line with financial procedures
  • Maintain Records: Keep accurate case notes and prepare reports, including court documentation when required
CANDIDATE REQUIREMENTS
ESSENTIAL
  • Professional qualification such as DIPSW, BASW, CQSW, or relevant qualification (e.g., Nurse, OT, Physiotherapist)
  • Registration with Social Work England (or relevant body)
  • Previous experience in adult social care or health settings
  • Proven experience of supervising and developing staff
  • Knowledge of Adult Care legislation and safeguarding procedures
  • Excellent communication, assessment and decision-making skills
  • Ability to build strong multi-agency relationships
  • Confident IT and record-keeping skills
