A local government organization in Oldham is seeking a Senior Practitioner for the Older People's Mental Health Team. This role involves undertaking assessments, developing Care & Support Plans, and acting as the Safeguarding Adults Manager within a multi-disciplinary team. Working within Oldham Council’s Older People’s Mental Health Team. You will be working within a specialist multi-disciplinary team supporting people with a severe and enduring mental health condition. To have a responsibility for a small, complex, caseload and fulfilling statutory duties such as undertaking assessments, developing Care & Support Plans, and completing reviews, including Section 117 reviews. To support the Team Manager with management responsibilities, including acting as the safeguarding adults manager, team supervision’s, contributing towards team development, authorising care plans and providing management oversight. To continue to develop professionally, and be accountable for the delivery of, and promote, social work good practice.
We are a friendly team who to prioritise team cohesion and wellbeing, having monthly supervisions, team meetings and peer reflective sessions. We also support CPD, providing protected time, with a commitment to support people through AMHP, BIA, Practice Educator, and Leadership programmes.
