Are you a Band 5 Physiotherapist ready to take the next step in your career? Or a Band 6 looking for a fresh, rewarding challenge this winter? We’re offering a fantastic opportunity to join our dynamic and supportive Intensive Rehabilitation Service (IRS) – a multidisciplinary community team that plays a vital role in reducing hospital admissions and supporting early discharges.

As a Senior Physiotherapist, you’ll:

  • Manage your own caseload of community-based patients
  • Deliver high-impact, person-centred rehabilitation in patients’ homes
  • Collaborate with inpatient and acute teams to ensure seamless transitions of care
  • Provide supervision, training, and joint sessions with colleagues across the team

This is your chance to make a real difference while gaining valuable experience in a fast-paced, community-focused setting.

If you’re passionate about rehabilitation, thrive in a collaborative environment, and want to grow your leadership skills – we’d love to hear from you.

Please review the Job Description and ensure you meet the requirements before applying.

Service Context and Responsibilities

Working with patient centred goal orientated rehabilitation to promote self-management and independence throughout the continuum of care. Providing onward referrals to other community services including health and social care to ensure the safe management of the patient when completing their IRS episode of care.

The post holder will be a Registered Physiotherapist (HCPC) responsible for patients allocated to them and will assess, manage, plan and deliver care, including being professionally and legally accountable. The post holder will support their peers and team leader and be an effective team member whilst working on their own initiative. They will have a commitment to collaborative working and actively support and liaise with other health and social care professionals and agencies.
