A community health services trust in Wokingham is looking for a Senior Physiotherapist to join their multidisciplinary team dedicated to supporting patient rehabilitation and independence. The role involves assessing complex cases, managing referrals, and providing compassionate care in a community setting. Candidates should have strong communication skills and relevant qualifications. The post holder is a Senior Physiotherapist (band 6) with the clinical skills, expertise and knowledge to support community reablement, day urgent, community Physio & Falls service.
Intermediate care is a multidisciplinary service that helps people to be as independent as possible. It provides support and rehabilitation to people at risk of hospital admission or who have been in hospital. It aims to ensure people transfer from hospital to the community in a timely way and to prevent unnecessary admissions to hospitals and residential care.
Wokingham Intermediate Care is a team of Physiotherapists, Occupational therapists, and Senior therapy assistants, who treat people in their own homes to rehabilitate them to as high a level of independence as possible. This supportive multidisciplinary team works with patients across Wokingham in close communication with social services, voluntary agencies, and GPs. This post involves 7 days working within a shift pattern within the intermediate service.
Complete the timely assessment and management of referrals with multiple co‑morbidities and complex rehabilitation needs.
