Job description

An exciting opportunity has arisen for an enthusiastic and dedicated Physiotherapist to join our integrated team based at Stansted Surgery, providing community physio to patients living in Uttlesford. We treat patients with a wide variety of conditions: neurological, orthopaedic, fallers, musculoskeletal and rehab patients, aiming to improve their function, safety and independence. The post holder must hold a full driving licence and have access to a car for work, will be a lone worker and manage an independent caseload with regular supervision.

The ideal candidate will have wide clinical experience and a desire to build on prior skills and knowledge. Excellent time‑management skills are needed to prioritise a busy caseload and a flexible approach is required to accommodate the changing needs of the service. Effective team working and good IT skills are essential.

This post would be an excellent opportunity for a current Band 6, a Band 5 with experience or a Return to Practice therapist, as we can provide support and supervised learning to achieve the required standard.

ROLE SUMMARY
  • The community team of West Essex is part of the wider Hertfordshire and West Essex Integrated Care System, supporting out‑of‑hospital care for West Essex residents.
  • The post holder will work as part of integrated community teams with a focus on rehabilitation, helping patients to engage independently in activities which enhance their health & wellbeing. Interventions will be carried out within patients’ own homes, 24‑hour care facilities and clinics.
  • The post holder will plan, deliver and evaluate all aspects of the patient treatment process and may include onward referrals to appropriate community‑based health and social care agencies.
KEY RESPONSIBILITIES
  • Deliver high quality, skilled clinical care to patients and provide rehabilitation and long‑term condition management in a variety of community settings.
  • Manage a caseload of patients with complex health & social care needs, using evidence‑based, client‑centred principles to assess, plan, implement & evaluate interventions.
  • Utilise specialist skills and knowledge to undertake comprehensive assessments of patients.
  • Analyse information gained from assessment to formulate accurate interventions.
  • Collaboratively discuss and agree SMART goals and outcomes for treatment with patients, using negotiation and motivation skills when necessary.
  • Ensure patients understand and consent to treatment plans. Work within the legal framework for patients who lack capacity.
  • Develop collaborative treatment programmes with patients, carers and colleagues, demonstrating client‑centred practice and specialist problem‑solving techniques.
  • Evaluate patient progress, re‑assess and review treatment programmes as required, determining potential for rehabilitation and future care needs.
  • Plan and prioritise own workload and delegate tasks where appropriate.
  • Facilitate safe transfer of patients using approved moving and handling techniques and equipment.
  • Assess and clinically justify the provision of equipment, ensuring safety and advising the multi‑professional team/family.
  • Identify and undertake risk assessments of patients and environments during each intervention.
  • liaise with GPs, other professionals and voluntary agencies to deliver patient‑centred care.
  • Hand over patients to non‑qualified staff, providing clear treatment plans and reviewing progress at regular intervals.
  • Manage discharges in a timely manner, completing clear discharge summaries that explain interventions, outcomes and next steps.
ADDITIONAL DUTIES
Communication Skills
  • Use verbal, non‑verbal and written communication skills to ensure patients and carers understand their condition and treatment aims.
  • Adapt communication techniques to individual needs of patients (e.g. learning disability, sensory problems, neurological difficulties, or non‑native English).
  • Effectively communicate with the multidisciplinary team.
  • Escalate concerns to senior staff in a timely manner, completing DATIX forms and safeguarding referrals as per Trust policy.
  • Represent the therapy service in meetings, actively contributing and reporting relevant information.
  • Maintain accurate clinical records in accordance with professional and Trust standards, producing timely electronic patient records and reports.
Professional Skills
  • Adapt interventions to meet individual patient needs, ranging from vulnerable to highly informed patients.
  • Take a holistic, patient‑centred view, being sensitive to culture, beliefs and emotions.
  • Be professionally and legally accountable for all aspects of own work, including patient management.
  • Participate or lead specific projects to enhance patient care and service delivery.
  • Supervise and support junior staff, assistants and students under line manager guidance, using formal and informal supervision and appraisals.
  • Manage own workload within working hours, completing clinical and non‑clinical tasks.
  • Actively participate in Continuous Professional Development, maintaining a portfolio of learning outcomes.
  • Take an active part in the therapy in‑service training programme, delivering training as required.
  • Evaluate own work and practices using Evidence‑Based Practice, participating in audits and outcome measures for service development.
  • Demonstrate understanding of Clinical Governance and Risk Management, applying this to work situations.
  • Deputise for therapy leads in their absence and cover service needs across localities as required.
  • Delegate tasks appropriately to assistants, carers and other professionals.
  • Be aware of Trust‑wide and local policies and procedures, national guidelines and legislation relating to health and social care.
  • Maintain confidentiality at all times, consistent with Trust policy.
EDUCATION AND RESEARCH
  • Maintain up‑to‑date knowledge and skills through professional development training.
  • Identify learning needs, providing training and mentoring to team members, students and health care professionals.
  • Provide clinical support to colleagues as required.
  • Participate in clinical supervision regularly in line with Trust guidelines.
  • Participate in annual appraisal and maintain a personal development plan.
  • Participate in service development to ensure innovative practice and maintain standards.
  • Complete mandatory training in line with Trust policy and procedures.
