Senior Physiotherapist – Community & Interim Care

Job summary

CPFT invites applications for a Senior Physiotherapist (Intermediate Care) based in Huntingdon. You will work within our expanding Intermediate Care Team, delivering community-based assessment, rehabilitation and discharge planning to support earlier hospital discharge.

The role requires autonomous practice within an MDT, with responsibilities across care-at-home and care-home settings, contributing to high-quality care and service delivery across Cambridgeshire and Peterborough.

Qualifications

• Degree or equivalent in Physiotherapy with HCPC registration.
• Documented evidence of continuing professional development (CPD).
• Member of CSP and evidence of registration.
• Experience in community/intermediate care settings is desirable.

Responsibilities

• Undertake assessments of patients with complex pathologies using specialist clinical reasoning.
• Identify patient needs, set goals and provide appropriate therapy interventions and rehabilitation programmes.
• Work across Pathway 1 (care at home) and Pathway 2 (care home beds) for the Intermediate Care service.
• Work as an autonomous practitioner within the integrated multi-professional team.
• Be responsible for patients on shared caseloads in community and/or care home settings.
• Provide holistic therapy interventions and support across professional boundaries.
