Job Summary

An exciting opportunity has arisen within the Woodbridge Community Team. We are looking for an experienced band 6 Physiotherapist who is passionate, committed, innovative, enthusiastic and highly motivated to delivering high quality care within patient's homes.

This is a permanent contract for 30 hours per week, with pattern of work to be negotiated.

We are fortunate enough to be co-located with our nursing and social care colleagues.

This is an opportunity to work within a knowledgeable experienced team and wider MDT.

You should be flexible, motivated and willing to work within a changing environment.

We will provide support through in-service training and professional forums, Mandatory training, 1:1 and personal development meetings as well as clinical rolling skills training via the practice development facilitators. We facilitate students on training and have close working relationships with neighbouring community teams in Health and Social Care.

Main Responsibilities
  • To provide high standard patient care in patients' homes.
  • To ensure a more seamless flow for patient care between hospital and community settings, enabling patients to be cared for in their own home environment.
  • To provide supervision and support to more junior staff.
  • To triage referrals to the community team.

Person Specification
Experience Essential
  • Experience of the supervision of staff and students.
  • Experience of participating in research and audit.
  • Evidence of skill relevant to the rehabilitation of adults with physical disabilities gained through theory and practical course and experience.
  • Experience of working with adults with complex physical, sensory and cognitive disabilities.
  • Experience of working with a range of primary and community services including Health, Social Care Services and the independent Sector.
  • Proven post registration experience within a multi-disciplinary service.
  • Leadership experience in a Health/Social care setting. Planning decision making skills.
Desirable Experience
  • Current professional student practice placement educator training and associated skills.
  • Membership of a special interest group relevant to the role.
Qualifications Essential
  • Diploma or Degree in Occupational Therapy/Physiotherapist.
  • Health Professions Council Registration.
Desirable Qualifications
  • Membership of an appropriate professional body.
Knowledge Essential
  • Documented evidence of Continuing Professional Development.
  • To demonstrate a knowledge of Health & Safety legislation for self and others.
  • Current knowledge and skills of moving and handling dependent adults and older people.
  • Knowledge of current manual handling regulations.
  • A clear understanding of, and commitment to clinical governance, health and safety and risk assessment.
  • A knowledge of primary and community services.
  • An in-depth knowledge and understanding of the NHS Plan, relevant NICE guidelines and NSF's relating to own and associated services and the implications for service provision. High level of skill in developing and maintaining effective working relationships with relevant stakeholders.
  • Knowledge and understanding of professional and ethical issues.
  • A specialist knowledge and application of current best practice in the rehabilitation of adults with physical disabilities.

  • Able to communicate effectively in writing and verbally using the English language.

  • Willingness and ability to travel across East Suffolk using own transport. A pool vehicle may be available.
