Aphysiotherapist who has an experience in clinical management of patients withcomplex health and rehabilitation needs, who are at risk of a deterioratingquality of life.

Someone keento work with other health, social and community services to ensure continuityof care on discharge.

The Physiotherapist/post holder isresponsible for providing a comprehensive and co-ordinated range of specialistphysiotherapy therapeutic and preventative services and rehabilitation programmes which maintain orimprove the health, independence, and quality of life our patients on Caterham Dene Ward andin the community, reflecting the philosophy of First Community Health and Care.
The post holderwill be part of our multi-disciplinary team, working across the ward and in thecommunity, to prevent unnecessary hospital admissions and facilitate timelyhospital discharges for patients and ongoing rehabilitation.
The post holderwill apply knowledge and skills across a range of conditions and medicalimpairments, procedures and practices gained through practical experience andunderpinned by theoretical knowledge.
The team provides a seven day a week rehabilitation service at CaterhamDene Hospital and in patients homes to enable safe rehabilitation. The post holder willparticipate in the weekend and bank holiday rota. Post holder will need to have a full drivers licence andcar for personal business use andbe able and prepared to drive between our bases at Caterham Dene Hospital andother community settings, as required, during their working day.
