Job summary

This is an exciting opportunity for an experienced pharmacy technician to join our Integrated Pharmacy workforce providing a service to our PCNs within Wallasey and Moreton and Meols. These PCNs are a group of 8 GP practices working together to provide joined up services to support their patients to live happier, healthier lives. You will join an experienced and dynamic team that includes GPs, Nurses, Health & Wellbeing Coaches, Care Coordinators, Social Prescribers & Physiotherapists to provide clinical pharmacy services to our local population.

Main duties of the job

The role will support the PCN clinical pharmacists to ensure the delivery of safe, effective systems for repeat prescribing, medicines optimisation, reducing medicines waste and maximising patient outcomes.

  • Be responsible for the development of a better understanding of the principles of medicines optimisation throughout the practice teams and promote good practice in line with clinical and operational guidelines.
  • Perform medicines reconciliation for patients discharged from hospital including counselling on medicines use and referral to other clinicians as necessary.
  • Communicate with a wide range of healthcare professionals within the trusts, practices and other services such as care home staff.
  • Build strong relationships with community pharmacy colleague.
  • Support patients with their medication and provide advice regarding benefits, risks and side effects.
  • Support improvements in patient care and the development of integrated pathways across primary and secondary care.

As part of the WUTH Integrated Pharmacy Service that supports a number of PCNs within Wirral, these roles are based within Wallasey Wellbeing and Moreton & Meols PCNs but may also support our other PCNs as required by service need. There are opportunities to work with colleagues from the other PCN teams in Wirral as well as having access to the Acute Trust specialists which helps you develop a better understand the full patient journey.
