Job summary

Qualifications

Significant post-registration and specialist palliative care experience.
Experience working in a community environment and managing a caseload.
Management experience including staff, budgets and service development.
Excellent communication and interprofessional collaboration.

Responsibilities

Provide advanced palliative and end-of-life care in community settings across Southern Hospice Group.
Assess, review and support complex cases with safe, effective care plans.
Offer guidance to families and carers and advise colleagues across primary and secondary care.
Lead handovers and ensure integrated working across multidisciplinary teams.
Promote patient-centred care and advocate for patient wishes.
Support training and development through MDT participation and education.
