Job summary

Leicestershire Partnership NHS Trust is seeking a Band 6 RMN for the Central Referral Hub/Unscheduled Care Service based at the Neville Centre, Leicester General Hospital. The role operates Monday to Friday 09:00–17:00 with travel minimised by remote working where possible.

The successful candidate will work within a multidisciplinary team, conduct mental health assessments, and support collaborative care planning and discharge planning to meet patient needs.

Qualifications

• Sound clinical skills and ability to formulate collaborative care plans.
• High standard of written and verbal communication.
• Experience working in mental health settings is desirable.

Responsibilities

• Assess mental health and social needs of service users.
• Coordinate care and treatment activities with MDTs and external agencies.
• Plan and discharge care with detailed, evidence-based plans.
• Ensure timely response to address immediate risk on a shift basis.
