Job Title:Senior Integrated Service Manager

Hours of work:Full time (37.5 hours per week Monday to Friday)

Responsible to: Head of Integrated Services

Accountable to:Director of Operations

Location:Primarily Haringey GP Federation Offices (48 Station Road, Wood Green, London, N22 7TY) - minimum of 3 days per week.

Main duties of the job

This role provides an exciting opportunity for an enthusiastic individual to lead our Integrated Services Team. These services address health inequalities for the vibrant and diverse population in Haringey and improve co-ordination with other health, care and voluntary sector providers. There are currently 9 services in our team which focus on

Long term conditions - supporting residents in identifying, treating and their confidence in managing these conditions themselves through proactive and anticipatory care.

Core2Plus5 and Vulnerable population groups including those living with severe mental illness, substance misuse and homelessness.

Reducing health inequalities i.e.. identifying patients awaiting hospital treatment or with other high-use of healthcare services to identify if their needs can be better managed through a multidisciplinary team approach.

It is expected that this range of services will grow over time and the post holder will work with our Head of Integrated Services to mobilise any new integrated services and retain oversight of existing services. This will include supporting the development of business cases, mobilisation plans and agreeing contracts, as well as, reviewing and improving existing services and associated processes.

Job responsibilities

The post holder will be responsible for leading all the teams who work in the above services, to deliver KPIs, and ensure the services meet the needs of our population and our member practices. This will involve reporting back to the Director of Operations and the Integrated Care Board (ICB) as part of their assurance process.

The postholder will have experience leading teams to deliver services. They will be able to inspire and support people. They will be solution focused and able to work collaboratively. They will have an excellent track record in delivering on projects and initiatives.

The line management responsibilities of the two service managers will sit with the Senior service manager. They will support development of the team and PDP to enable the team to develop in their roles. They will support the head of integrated services to deliver the objectives of each service but be closer to personnel development and developing the maturity of the integrated team models. The service development and operational elements of the service will be weighted towards the role of the Head of Integrated service manager.

The senior service manager will on occasion work directly with the Director of Operations on specific change management and service improvement activity where necessary.

Qualifications
  • Degree level qualification or equivalent experience
  • Evidence of continuing personal development
  • Project and/or Programme Management Qualification (PRINCE2 or equivalent)
  • NHS Leadership or Educational Qualification
Experience
  • Experience of leading the planning and delivering of programmes involving a wide range of stakeholders with delivery on time and within budget.
  • Experience of managing services, reporting against performance and ensuring high quality care is delivered.
  • Successful partnership working across different organisations and sectors
  • Previously responsible for a budget with a working knowledge of financial processes
  • Experience of writing bids and business cases
