NHS Lanarkshire is recruiting a Diabetes & Adult Weight Management Programme Co-Ordinator to lead delivery of DSD and AW M services across Lanarkshire. The role aligns with national public health frameworks and local priorities, driving multi-tiered health improvement initiatives to prevent ill-health.

Qualifications

• Qualified to degree standard.
• Postgraduate qualification in health improvement/health promotion/public health or equivalent.
• Knowledge of public health competences or UKPHR registration.
• Awareness of local and national policies affecting population health.

Responsibilities

• Provide leadership to initiate, develop, deliver and monitor health improvement plans and programmes.
• Agree annual objectives with the Unit Health Improvement Team Leader and conduct reviews.
• Provide health improvement advice to NHS staff and partner agencies to promote evidence-based practice.
• Analyse locality population health data to inform programme delivery changes.
