Job summary

Royal Surrey NHS Foundation Trust in Godalming is seeking a Senior Health Care Support Worker (Band 3) to join the Milford Integrated Care Hub in outpatient services. Are you an experienced Health Care Assistant looking to develop skills with a friendly community team working within an outpatient department?

• As part of the Royal Surrey Foundation Trust, both PACH and MIC Hub offer you the opportunity to work for a unique, dynamic organisation that’s transforming adult community services locally.
• Working as part of the MIC Hub team you will work closely with nursing colleagues reviewing patients coming in for a variety of appointments including frailty clinics, Parkinson’s clinics and IV clinics within the department.
• You will be responsible for completing assessments for patients including, but not limited to, observations, ECGs, bladder scans, cognitive assessments and blood tests.
• You will manage your own patients arriving, under the supervision of our registered nurses team and we will support developing your skills within the team.

MICH: The Milford Integrated Care Hub is run as outpatient department and provides diagnostic process to determine the medical, psychological and functional capability of someone living with frailty. By providing holistic assessment for older people with complex needs, aim is to improve the patient’s quality of life, promote self-care, reduce risk of deterioration in patient’s condition and avoid unnecessary hospital admission.

Main duties of the job

The Band 3 Health Care Support Worker will carry out delegated tasks to support the delivery of high-quality patient care within the Milford Integrated Care Hub. The role involves providing direct care to patients and offering day-to-day support and guidance to junior staff.

• To support and guide Band 2 Health Care Support Workers and students, contributing to their development and promoting a positive learning environment.
• To work as part of the multidisciplinary team under the supervision of registered nurses, undertaking delegated clinical and non-clinical tasks within agreed competencies.
• To demonstrate a developed level of knowledge, skill and initiative appropriate to a Band 3 role, contributing to the safe and effective delivery of care within the outpatient setting.
• To support the effective organisation and flow of outpatient clinics, ensuring a safe, efficient and patient-centred environment.
• To recognise changes in a patient’s condition and escalation concerns appropriately to registered staff.
• To undertake the Trust’s in-house training for Healthcare Support Workers and maintain required competencies relevant to the role.
• To assist in the induction, supervision and support of Band 2 Health Care Support workers and new staff members as directed.

Detailed job description and main responsibilities

• To provide high-quality, patient-centred care and promote positive health and wellbeing.
• To undertake and accurately record clinical observations, including temperature, pulse, respiration, oxygen saturation, blood pressure, weight and blood glucose levels, reporting any abnormalities promptly to a registered nurse.
• To undertake routine urine testing, record results and escalating abnormalities appropriately.
• To undertake training and demonstrate competency in a range of delegated clinical skills, including spirometry, ECG recording, phlebotomy and basic wound care, within agreed competencies and under the direction of registered staff.
• To perform basic and less complex dressings using aseptic non-touch technique (ANTT), following training and competency assessment.
• To observe and report changes in a patient’s condition, including skin integrity, escalating concerns appropriately.
• To maintain awareness of infection prevention and control measures, including standard precautions, and apply these in practice.
• To respond appropriately in emergency situations following training, summoning assistance and supporting registered staff as required.
• To adhere to Trust policies relating to moving and handling, health and safety, and risk management.
• To complete clear, accurate and contemporaneous documentation in line with care provided and Trust standards.
• To support the effective organisation and flow of outpatient clinics, assisting across clinical areas to maintain a safe and efficient service.
• To maintain stock levels of consumables, including ordering and replenishing supplies as required.
• To support and guide Band 2 staff, students and new team members in the delivery of care, contributing to a positive learning environment.
• To take responsibility for own actions, recognising the boundaries of the role and working within agreed competencies.
• To undertake all mandatory and role‑specific training and maintain required competencies.
• To contribute to clinical governance activities, including audit, incident reporting, quality improvement and participation in meetings as required.
