We are seeking a highly skilled and experienced Senior Clinical Pharmacist to join our busy GP practice. The successful candidate will report directly to the Lead Clinical Pharmacist and the Medicines Management GP Partner. You will work collaboratively with a dynamic team of pharmacists and a pharmacy technician to deliver exceptional patient care and optimize medication management.

Medication Reviews: Hold clinics for patients requiring face‑to‑face structured medication reviews, provide telephone support, undertake structured reviews with multi‑morbidity and polypharmacy patients, implement prescribing changes (as an independent prescriber) and order relevant monitoring tests.
Long Term Conditions: See patients in long‑term condition clinics, implement improvements to patients’ medicines, including de‑prescribing, manage own case load, run own long‑term condition clinics with GPs and support clinical decision making.
High‑Risk Medication Reviews: Manage patients at risk of hospital admissions and harm from poor use of medicines, attend and refer patients to multidisciplinary case meetings.
Repeat Prescribing: Produce and implement a practice repeat prescribing policy, manage the repeat prescribing re‑authorisation process, review medicines reaching review dates, ensure patients are booked in for necessary monitoring tests where required.
Medicines at Discharge from Hospital: Reconcile medicines following discharge, identify and rectify unexplained changes, perform post‑discharge medicines care plan, dose titration, booking of follow‑up tests, work with patients and community pharmacists.
Medicines Safety: Work with patients, network and primary care teams to minimise risks through medicines optimisation, review national/local policy and guidance, manage the implementation of changes to medicines and guidance, develop protocols for managing patients on high‑risk medications.
Cost Saving: Recommend and manage pharmacy technicians to make changes to medicines (switches) designed to save on medicine costs.
Clinical Advice and Expertise on Medicines: Answer all medicine‑related enquiries from GPs and practice staff, provide telephone advice, monitor effect of changes.
Care Quality: Ensure the practice(s) are compliant with CQC standards where medicines are involved.
Training: Provide education and training to practice staff on therapeutics and medicines optimisation.
