Central London Community Healthcare is seeking an Emergency Care Practitioner for a 12-month fixed-term contract. You will provide high-quality care in home and community settings, using advanced assessment and autonomous decision-making to manage acutely unwell patients.

Role requires leadership, collaboration across pathways, and delivery of advanced clinical interventions. You will work across a 7-day rota (08:00–23:00) supporting safe caseload management, discharge planning and service

Qualificações

• Registered nurse or paramedic with current registration and post-registration development.
• Advanced clinical assessment and decision-making relevant to urgent care.
• Experience in primary, community or urgent care settings.

Responsabilidades

• Provide autonomous clinical care to acutely unwell patients at home or in community settings.
• Assess referrals, prioritise by clinical need, and respond within agreed timescales.
• Lead and supervise junior colleagues; contribute to service development and quality improvement.
• Collaborate with multidisciplinary teams to ensure safe patient pathways and continuity of care.
• Deliver advanced clinical interventions including venepuncture and cannulation as required.
• Support admission avoidance and timely discharge from hospital-based care.
