Job summary

HCRG Care Group’s Affinity Team is recruiting a senior practitioner to support Hertfordshire's Early Intervention and Prevention Service for children and young people at risk of escalation. The post involves managing a direct caseload and coordinating multi-agency formulation work across education, community and family settings countywide.

The role requires trauma-informed practice, safeguarding knowledge, and excellent communication with children, families and partners.

Qualifications

• Significant experience working with children, young people and families in a relevant service.
• Experience with Framework for Integrated Care and/or Rule of 167.
• Experience facilitating multi-agency planning, Team Around the Family meetings or network meetings.
• Experience delivering 1-1 interventions to children, young people, parents or carers.

Responsibilities

• Guide and support EIS Practitioners so the service model is delivered consistently and effectively.
• Hold and manage a direct caseload from assessment through to closure, keeping records accurate and contributing to outcome tracking.
• Facilitate formulation-led practice with the child, family and professionals to build shared understanding.
• Coordinate multi-agency formulation discussions and align plans around the child.
• Provide senior input on complex cases and elevate risks or service decisions to the Affinity Team Lead.
• Build working relationships across education, CAMHS, Mental Health Support Teams, social care, early help, youth justice and voluntary sector partners.
