Job summary

A healthcare organization in North Wales is seeking an experienced Registered Band 6 Practitioner to join their dynamic multi-disciplinary team. In this role, you will hold a caseload as Care coordinator, delivering rehabilitation-focused services for individuals with complex emotional needs in community and inpatient settings. Ideal candidates will have a professional qualification in Adult Mental Health, experience in rehabilitation management, and effective communication skills. The ability to speak Welsh is desirable, but not mandatory.

Qualifications

• Professional qualification in mental health or related field required.
• Evidence of continued professional development necessary.
• Experience in mental health assessment and rehabilitation management.

Responsibilities

• Hold a caseload and act as Care coordinator under the Mental Health (Wales) Measures Framework.
• Provide rehabilitation focused services for users.
• Supervise and manage junior staff members.
