A leading healthcare provider in Woking is seeking a Senior Community Rehabilitation Assistant to join their Urgent Community Response service. The role involves preventing hospital admissions, managing a caseload, and being part of a supportive multidisciplinary team. Candidates should have NVQ levels 1 and 2 in Health and Social Care or equivalent and a passion for community health. We are looking for enthusiastic and motivated individuals with experience in health and social care to join our highly regarded Urgent Community Response (UCR) Service, based at Woking Community Hospital. We provide a 24-hour, 7 Day urgent responsive NHS community services to patients across Northwest Surrey as part of an integrated service with Surrey Adult Social Care Teams. The Out of Hours UCR Team work overnight forming part of the 24-hour UCR service and there is flexibility within the UCR team to work across days, nights and to do internal rotation.
Working as a member of the multi-disciplinary UCR team you will work with patients within an agreed level of competence, who may require management and/or rehabilitation interventions in their own environment to prevent acute hospital admissions and to support complex hospital discharges.

To work as a Senior Community Rehabilitation Assistant, as part of the home-based Urgent Community Response (UCR) service, aiding rehabilitation and preventing hospital admissions.
To work both independently and as part of a team managing a delegated caseload, under the guidance and supervision of a qualified professional e.g. Physiotherapist, Occupational Therapist, Nurse and paramedics. To assist the UCR coordinators in maintaining the smooth running of the service and to work towards achieving a high quality and effective Urgent Community Response service, by being a proactive and integrated member of the team.
Providing a seven-day service within recognised shift patterns which can include being part of the Day service or part the Out of Hours (OOH) service as UCR provides support over 24 hours, 7 days a week
